Anatomy of the face
The face is made of multiple layers: the skin, an extracellular matrix, and muscle. The extracellular matrix is made up of elastic and collagen fibers, which keeps the skin elastic. The muscle layer is made of a combination of muscle and tissue and is also known as the SMAS (superficialmusculo-aponeurotic system). Underneath the skin of the neck is a muscle layer called the platysma, which acts like a sling to keep the neck taut and youthful.
As aging occurs, the skin on the face loses collagen and elastin due to sun exposure, gravity, and constant muscle contraction caused by talking, smiling, laughing, etc. The skin loses its elasticity causing deep wrinkles, jowl formation and sagging skin in the face and neck.
The best way to prevent photoaging or the breakdown of elastin and collagen is prevention. What is a face lift?
The primary goal of face lift surgery is to tighten tissues, which have started sagging due to age and gravity. A face lift will tighten the jowls and cheeks, the skin of the neck, tighten the neck tissues, and soften the smile lines. Advances in surgical techniques and materials allow tailoring face lifts for individual patients.
Some surgeons performing facelift in Houston only tighten the skin. The skin itself has no strength, and as a result these types of facelifts do not last more than a few months. At Athré Facial Aesthetics, all face lifts lift the muscle and tissue layer, leaving no tension in the skin. Therefore, the effect lasts longer and the incisions are hidden for a flawless final outcome. How is a face lift performed?
The incision generally is made in the hair around the temples and around the ear. A small (less than 1/2″) incision is made right under the chin to sculpt and tighten the neck tissues. After the skin is lifted, the underlying muscle layer is tightened and the excess skin is trimmed. Meticulous technique, attention to detail, and microscopic sutures all provide for a natural, non-operated look.
The goal is to restore a very natural look and to have patients look like themselves, just refreshed. No two patients are identical, so no two face lifts Dr. Athré performs are the same. Tailoring the surgery for each patient is the only way to prevent a “pulled” or windblown appearance.
Face lifts are performed under sedation or general anesthesia. A face lift can be combined with other procedures such as eyelid surgery, fat transfer, or even brow surgery. The surgery itself takes approximately 3.5 hours, and patients should be back to work in 7 to 10 days.

What is the RT Lift?

What makes the RT Lift special?
There are numerous mini facelift procedures for Houston residents to consider. These procedures attempt to reverse the signs of aging by just tightening the skin, or by using barbed sutures in the face, or by trying to suspend the face with permanent sutures. The results in these procedures are short-lived because these procedures do not address the muscle layer (SMAS) like a traditional face lift. The RT Lift tightens, repositions, and re-drapes the SMAS just like a traditional face lift, thereby giving the patient a fresh new look that lasts.
How is the RT Lift performed?
In general, the technique used for the RT Lift is the same as in a face lift. The incision is hidden in the hair above the ear, is then camouflaged in the ear canal, and then gently wraps around the ear lobe and then disappears in the crease behind the ear. The only difference between a traditional face lift and the RT Lift is that the RT Lift addresses a smaller area of concern. Patients with significant aging and a very heavy neck may be better served by a traditional face lift. This procedure is often performed with other facial enhancement procedures such as eyelid surgery in Houston.
What can RT Lift patients expect?
RT Lift patients have their procedure done on an outpatient basis. Because incisions are smaller, bruising and swelling are drastically reduced. Most patients are back to work within 5 days, and are on their feet the very next day.
Fat grafting is another way to fill facial features, add volume, and rejuvenate the face and it uses the patient’s own fat. The fat used for the fat transfer is extracted from parts of your body like the abdomen or thighs and injected into another area that requires plumping. Since the fat is your own, a significant portion of the fat becomes permanent, and therefore it eliminates the need for constant re-injections, as with other materials.
Fat grafting by itself is not a new procedure. However, the technique that Dr. Athré uses is. Traditional fat grafting had the advantages of a possible permanent rejuvenation solution, but had the disadvantages of granuloma formation, lumpiness, a necessity for overcorrection, a necessity of usage of large injection cannulas, etc.
Dr. Athré now uses the ALMI technique (https://www.almiprocedure.com/) and is one of the few surgeons in Houston, and in Texas to do so. This technique uses the same fat harvesting technique. However, the fat is then micronized and reconstituted with PRP (platelet rich plasma). The combo material can be injected in much the same way as Restylane® or Juvederm® through tiny needles. This technique reduces many of the complications such as irregular contours and lumpiness seen in older techniques. Finally, the initial results of this technique show that the additional volume can last for longer than 5 years!

The eyebrows help shape the overall look of the face and frame the eyes. The eyes are one of the most important focal points of the face; and therefore, aging in the region of the eyes ages the entire face.
As aging naturally occurs, the eyebrows gently start to fall. This gives the appearance of thickness around the upper eyelid. As the eyebrows start to fall, forehead wrinkles and deep wrinkles between the eyebrows can form. Anatomy & Aging of the eyebrow region
Underneath the skin of the forehead is a tissue layer and a muscle layer. The muscle layer, or the frontalis muscle, holds the brows up. This muscle is used during squinting or if surprised. The tissue layer is made up of a matrix of collagen and elastin, which gives the forehead youthful fullness.
With aging, the brows slowly start to fall due to gravity and the loss of collagen. The frontalis muscle works harder to hold the brows up, causing forehead wrinkles. Fine wrinkles start to form due to the breakdown of elastin and collagen fibers within the tissue matrix of the skin. The skin around the forehead can appear rough with uneven texture and color.
The primary nonsurgical therapy for aging of the brows is Botox®. BOTOX® can be used to address mild aging of the brows and forehead wrinkles by relaxing the frontalis muscle. If injected carefully, BOTOX can relax the forehead lines and elevate the brows slightly for up to 3 to 4 months per injection. BOTOX cannot be used to treat moderate or severe brow aging.
Surgical approaches to the brow elevate the brow by lifting the tissue and the muscle. There are several techniques such as a coronal brow lift, endoscopic brow lift, direct brow lift, and many others. The choice of technique is tailored to each patient after taking into consideration the patient’s anatomy, gender, natural brow shape and ethnic considerations. If you have noticed some sagging, excess tissue in your neck or a loss of definition of the neck angle, surgical retightening is possible from Dr. Raghu Athré. The neck is made of skin, a collagen-rich matrix layer, and a deep muscle layer. The muscle layer, or platysma, envelops the neck and acts like a sling to support the tissues of the neck. Over time, one or more of these tissue layers may lose elasticity, making a surgical neck lift or neck liposuction necessary for Houston area residents.
Aging and the neck
Gravity and genetics are the key components that cause signs of aging in the neck. As aging occurs, gravity pulls on the neck tissues causing them to droop and fall. Also, the collagen and elastin-rich matrix of tissue deep in the skin slowly degrades. These actions provide less support to the tissues of the neck, and over a lifetime, loose tissue, also known as a “waddle,” can form in the neck.
Genetics also plays a major role in aging of the neck and in the development of excess fat in the neck. The end result is loose skin in the neck, excess soft tissue and fat in the neck, and loss of the sharp, youthful angle of the neck.
Botox® can be used to reduce the appearance of platysmal banding or the vertical lines in the middle compartment of the neck. The BTL Exilis Ultra laser can be used to: (1) reduce fat in the central compartment, and (2) tighten the collagen matrix of the neck to enhance the neck contour.
Overcoming concerns with the neck is predominantly addressed through surgical approaches. A little excess fat and mild droopiness of the neck tissues can be effectively removed with liposuction. Liposuction removes the excess fat and tightens the tissues of the neck. It is commonly performed in conjunction with RT Mini Face Lift, but can be performed alone. The purpose of liposuction is not only to remove excess fat, but also to trigger a platysma muscle contracture. This tightens the neck and sharpens the neck/chin angle.
Significant excess neck tissue and excess fat must be addressed with a combination of liposuction and neck lifting. The neck lift is most often performed with a full face lift, but can be performed independently as well.
These procedures are usually performed under local anesthesia, deep sedation, or general anesthesia. The procedure itself takes 30 minutes to an hour. Patients are able to go home the same day, and are able to return to full physical activity within 1 week.
The chin and the cheeks are the bony prominences that bring the entire face into proportion. Having a recessed chin can give the appearance of an unbalanced face, an overly projected nose, or even a lack of definition in the neck area. A recessed cheek area can make the middle one-third of the face seem small and unbalanced. Chin and cheek augmentation from Dr. Athré is a simple, quick procedure to add volume to the cheek and/or chin area.
The chin is made up of skin, soft tissue, and bone. As aging occurs, the bone of the jaw slowly recedes, causing a smaller chin and small grooves to occur in front of the jowls. A mini facelift can help lift the jowls, but may not address the small groove in front of the cheeks. This groove is due to the loss of bone in the jaw that occurs with aging. Furthermore, aging or genetics can cause a small midface. This can cause the cheek fat pad to hang lower and cause premature aging.
Chin augmentation is the primary way to addressloss of volume in the chin area. The procedure can be performed in the clinic, in the operating room, or in conjunction with other procedures. Careful examination and measurements are taken during the initial consultation to help determine which technique is the most suitable.
The procedure takes 30 minutes, and patients can return to full physical activity the next day. Chin augmentation is commonly done in conjunction with to bring the face into aesthetic proportion while bringing the nose into an aesthetically pleasing profile and proportion. Chin augmentation is also done in conjunction with facelifts or the RT Lift to add volume to the chin, sharpen the neck angle, help eradicate the jowls, and erase the grooves that can sometimes form in front of the jowls.
Cheek augmentation can elevate the cheek fat pad and add volume to the cheek area. The procedure can be done under local anesthesia with sedation, or under general anesthesia. The procedure normally takes 1.5 hr and patients can return to work in approximately 1 week.
