 40 years old. “Speed bumps” developed around the mouth. Which procedure?

Is it fat deposits or loose skin that creates the “speed bump” on either side of my mouth? The second photo is an example of a smooth face, clean, defined jawline that I am hoping for. Which procedure is right to achieve this look: buccal fat removal? chin liposuction? mid-lower face lift? fillers/fat transfer only? I will have tear troughs and cheek fillers done, which should help improve the overall look. I appreciate any feedback you can provide. I am willing to travel for the right surgeon :).

The laxity that you describe is caused by the melolabial fold and the pre-jowl sulcus.  You have numerous options availaIble to you, and your overall treatment plan may require more than one solution.  Based solely on your photos, and my biases, I would…

  1.  Perform a mini lift to tighten the SMAS muscle/platysma and elevate the malar cheek pad.  This would rejuvenate the midface, sharpen the jaw line, give you fuller cheeks, and sharpen the neck line.
  2. If you wanted further softening of the melolabial folds, I would then add volume–fillers or fat.
  3. Skin tightening (skincare, poss peel, poss lasers)
